Coronavirus Update and Assistance

At Ke Ola Mamo, the safety and well-being of our employees, clients, and patients is always our priority.

Ke Ola Mamo will be closed to all walk-ins until further notice. The ability to continue and provide our community with the necessary primary, behavioral, and community healthcare services during this time remains essential.

Patient Registration

Please arrive 10 minutes early for your first appointment to allow time to complete all patient registration information.

To get ahead start, you may download and print the registration forms online. Be sure to have the form signed and dated, and bring form to your appointment.


What to Bring

At the time of the appointment you will be asked to:

  • Complete and sign a form indicating whether you have other health insurance.

  • Update your information every six months or whenever coverage changes.

Each time you visit one of our providers you will be asked to provide:

  1. Any printed Registration Forms

  2. Government-Issued Identification:

    • State of Hawaii Drivers License

    • State ID

  3. Current Insurance Card

  4. Facial Covering

  5. COVID-19 Vaccination Card


Additional Documentation

Depending on your needs, you may need to bring additional documentation:

  1. Legal Guardianship Papers

  2. Proof of Income for Sliding-Scale Price Application, which can include:

    • Pay Stub (Calculate Average per Month)

    • Determination Letter from Department of Human Services (Medicaid Recipient)

    • Social Security Benefit Letter

    • Unemployment Insurance Pay Stub

    • Current Tax Return

    • Disability Award Letter

  3. Shot Records/Other Health Records

  4. To get WIC, you need to bring:

    • Proof of income such as pay stubs, tax return or medical benefit papers

    • Proof of identity such as a state ID card or birth certificate for you and each child

    • Proof of Hawaii address such as a utility bill or rent receipt. All children you want to receive WIC


If you are uninsured, your share of the cost will be determined based on your family size and income.

If you are insured, Ke Ola Mamo will submit claims to your insurance company. As a result your insurance company may require us to collect a co-payment at the time of your appointment.

We will work with you should you be unable to pay your entire share. If you have any questions or concerns about your share of cost, please ask when you enroll or make a medical appointment.

As a patient of Ke Ola Mamo it is important for you to understand your responsibility:

Ke Ola Mamo operates on a Sliding-Scale Fee basis. This means that all services will be provided at a fee rate determined by your personal family income. All prices for services are subject to change based on your personal family income level. To be eligible for our sliding scale, we request all patients bring in a form of proof of income. This must be renewed on a regular basis.

We accept most insurance programs.
